Yersinia Infections
* Yaws is a tropical infection of the skin, bones and joints caused by the
spirochete bacterium Treponema pallidum pertenue. The disease begins with a
round, hard swelling of the skin, 2 to 5 centimeters in diameter. The center may
break open and form an ulcer. This initial skin lesion typically heals after
three to six months. After weeks to years, joints and bones may become painful,
fatigue may develop, and new skin lesions may appear. The skin of the palms of
the hands and the soles of the feet may become thick and break open. The bones
(especially those of the nose) may become misshapen. After five years or more
large areas of skin may die leaving a scar. Yaws is spread by direct contact
with the fluid from a lesion of an infected person. The contact is usually of a
non-sexual nature. The disease is most common among children, who spread it by
playing together.
* Yersinia is a genus of bacteria in the family Enterobacteriaceae. Yersinia
species are Gram-negative, rod-shaped bacteria, a few micrometers long and
fractions of a micrometer in diameter, and are facultative anaerobes. Some
members of Yersinia are pathogenic in humans; in particular, Y. pestis is the
causative agent of the plague. Rodents are the natural reservoirs of Yersinia;
less frequently, other mammals serve as the host. Infection may occur either
through blood (in the case of Y. pestis) or in an alimentary fashion,
occasionally via consumption of food products (especially vegetables,
milk-derived products, and meat) contaminated with infected urine or feces.
Infection with Y. enterocolitica can cause a variety of symptoms depending on
the age of the person infected. Common symptoms in children are fever, abdominal
pain, and diarrhea, which is often bloody. Symptoms typically develop 4 to 7
days after exposure and may last 1 to 3 weeks or longer. In older children and
adults, right-sided abdominal pain and fever may be the predominant symptoms,
and may be confused with appendicitis. In a small proportion of cases,
complications such as skin rash, joint pains, ileitis, erythema nodosum, and
sometimes septicemia, acute arthritis or the spread of bacteria to the
bloodstream (bacteremia) can occur.
